Psoriasis, Steroids, Sebum, and T-Immune Cells
What Do Onions Add to This Story?

Psoriasis is a chronic and troubling form of skin inflammation characterized by itchy red and scaly papules and plaques. It is primarily caused by mold allergy, food allergy, mold toxins, gut fermentation, and adverse food reactions. Stress worsens the condition. It begins with cellular fermentation and inflammation in the skin and forms patches of red rash which progresses to the formation of whitish scaly plaques. It is often associated fungal infections of toes. With time, skin inflammation is accompanied by inflammation of joints, tendons, and muscles, and the bowel. Some patients progress to colitis.

Steroids

Steroids are very effective and dangerous drug. They are effective in that they usually produce dramatic control of immune-inflammatory disorders, including psoriasis. They are and dangerous because they do not address causes of fermentative-inflammtory-immune disorders, and so deepen the real problem. People with eczema learn effective steroids are within days but usually recognize their dangers only after months of use.

Sebum Barrier

Sebum is oily substance produced in specialized sweat glands called sebaceous glands. It is secreted onto the outermost layer of the skin lining cells, and forms a barrier to microbial invasions of the skin and underlying tissues. Sebum strongly stimulates a type of skin immune cells called self-activated T Cells (self-motivated seems like a more appropriate designation for them). It is rich in some immunity-enhancing natural compounds, such as squalene.

Under ordinary conditions, sebum is not in contact with the deeper layers of the skin that contain T cells. However, in psoriasis prolonged disruption of the “sebum barrier” can bring self-motivated T cells into action and trigger fermentation, inflammation, and immune activation.

T Cells

T cells belong to a family of white blood cells called lymphocytes and play crucial roles in the immune recognition of injurious elements (microbes, chemicals, and others) in the immune responses launched to fight them. Populations of self-motivated T cells (self-reactive, in technical terms) reside in or near the surface layers of the skin and the gut.

Peeling An Onion Layer-by-Layer

Why bring onions into this discussion of psoriasis, steroids, and T cells? Because onions help me create a clear image of healing for my patients. Fermentation and inflammation of psoriasis needs to be peeled away layer by layer, just like an onion. Steroids create an illusion of healing, while onions bring home the reality of slow and steady clearing of layers of inflammation—layer by layer by layer, systematically recognition and elimination of causative factors.

As Roots Are to Roses, So the Bowel to the Skin

Death begins in the colon, so goes an ancient saying. In 29 years as a hospital pathologist, I examined over 14,000 colon and over 2000 skin biopsies. I have practiced integrative medicine for about thirty years. The core lesson my microscope and patients taught me is: as roots are to roses, so the bowel to the skin. The right approach to healing psoriasis is to detect and effectively all factors that trigger and perpetuate psoriasis lesions in the skin. This is as true of psoriasis as it is of eczema, acne, chemical dermatitis, and other disorders.

Self-Motivated T Cells Trigger Psoriasis

I return to the subject of T cells. Fatty substances of sebum fight microbes when when present in the right places and in optimal amounts. This system breaks down when these sebum fats permeate into wrong places and trigger self-motivated T cells called belonging to the family called CD12a cells. This was reported in 2014 by de Jong and colleagues in 2014 in an article published in the journal Nature Immunol (2014;15:177–185)
